The ABC’s World Today radio programme reports that a new pilot study by Rebecca Pang, Peninsula Health Victoria and Dr Nadine Andrew, National Centre for Healthy Ageing has found that hospital re-admission rates were cut by 66 per cent if patients who needed a bit of extra support had a regular phone call from a nurse after getting home.
