Southern Melbourne Primary Care Partnership (SMPCP)

Primary Care Partnerships like us are made up of a diverse range of member agencies who work together to improve local outcomes in hospitals, disability, drug and alcohol problems and women’s health.

Southern Melbourne Primary Care Partnership (SMPCP) formed in July 2014 as the result of a merger between Inner South East Partnership in Community Health (ISEPiCH) and Kingston Bayside Primary Care Partnership (KBPCP).

We cover the local government areas of Bayside, Glen Eira, Kingston, Port Phillip and Stonnington.

SMPCP delivers better health outcomes and stronger communities through:

The Victorian Government established the Primary Care Partnership Strategy in 2000 to strengthen the primary health sector and improve health and wellbeing outcomes across the service sector.
